KSM-66 Ashwagandha (Withania somnifera)

Ashwagandha is a cornerstone adaptogenic herb used for over 3,000 years in Ayurvedic medicine. KSM-66 is a branded, full-spectrum root extract produced through a unique water-based extraction process that preserves the full range of bioactive compounds. Standardized to a minimum of 5% withanolides, KSM-66 is the most extensively studied ashwagandha extract available, with over 24 published clinical trials.

How It Works

Ashwagandha exerts its adaptogenic effects primarily through modulation of the hypothalamic-pituitary-adrenal (HPA) axis, the body's central stress response system. When chronic stress keeps cortisol elevated, the HPA axis becomes dysregulated, leading to fatigue, anxiety, disrupted sleep, and impaired immune function.

Withanolides, the primary active compounds in ashwagandha, appear to work through several mechanisms. They modulate cortisol production by influencing the activity of cortisol-synthesizing enzymes in the adrenal glands. They also interact with GABA-A receptors, which may explain the anxiolytic effects observed in clinical studies. Additionally, ashwagandha has demonstrated effects on serotonergic pathways and may support healthy thyroid hormone levels by modulating thyroid-stimulating hormone (TSH).

At the cellular level, ashwagandha promotes the expression of heat shock proteins and antioxidant enzymes, which protect cells from stress-induced damage. This multi-target mechanism distinguishes adaptogens from single-pathway anxiolytics.

Clinical Evidence

The evidence base for KSM-66 is substantial. A 2012 randomized controlled trial by Chandrasekhar et al. demonstrated that 300mg twice daily of KSM-66 reduced serum cortisol levels by 27.9% and significantly improved scores on the Perceived Stress Scale over 60 days compared to placebo.

A 2019 study by Lopresti et al. found that 240mg of KSM-66 daily improved sleep quality, reduced sleep onset latency, and decreased anxiety in adults with self-reported sleep difficulties. Participants reported improved morning alertness without daytime drowsiness.

Salve et al. (2019) published a dose-response study showing that both 250mg and 600mg daily doses of KSM-66 significantly reduced cortisol and perceived stress, though the higher dose showed greater reductions in Hamilton Anxiety Scale scores.

For thyroid function, Sharma et al. (2018) found that 600mg daily of ashwagandha root extract normalized TSH and T4 levels in participants with subclinical hypothyroidism over 8 weeks, though this area requires more research.

A 2021 systematic review and meta-analysis by Bonilla et al. confirmed statistically significant effects of ashwagandha supplementation on reducing anxiety and cortisol levels across multiple randomized controlled trials.

Dosing and Safety

The clinically studied dose range for KSM-66 is 250-600mg daily, with most trials using 300mg twice daily or 600mg once daily. Effects on stress markers typically appear within 2-4 weeks, with full adaptogenic benefits developing over 6-8 weeks of consistent use.

Ashwagandha is generally well tolerated. The most common side effects are mild gastrointestinal discomfort, which often resolves with food. Taking ashwagandha with a meal improves tolerance and may enhance absorption.

Important contraindications include thyroid conditions (ashwagandha may alter thyroid hormone levels), autoimmune disorders (it may stimulate immune activity), pregnancy and breastfeeding, and concurrent use of immunosuppressants, sedatives, or thyroid medications. Individuals scheduled for surgery should discontinue use at least two weeks prior due to potential effects on anesthesia and blood sugar regulation.

Rare cases of liver injury have been reported with ashwagandha supplementation, though causality has not been definitively established. Those with pre-existing liver conditions should exercise caution and consult a healthcare provider.
